Key Responsibilities * Deliver high-quality social work services in accordance with relevant legislation, Council policies, procedures, and the Code of Practice for Social Care Workers. * Work collaboratively within a multidisciplinary team to achieve positive outcomes aligned with the Department of Health’s Adult Social Care Outcomes Framework. * Complete: * New adult social care assessments * Safeguarding enquiries * Reviews of care and support needs * Support the management team with the effective handling of incoming referrals and daily operational demands. * Maintain accurate and timely case recording and assessments.
