Team Manager - Hospital Discharge

Team Manager - Integrated Hospital Discharge Service £43.00 per hour | ASAP Start Hackney Council is currently recruiting for an experienced Adults Team Manager to join the Integrated Hospital Discharge Service. This is a key leadership role within a fast-paced, multi-disciplinary environment, focused on ensuring safe, timely, and effective hospital discharges for Hackney residents. About the Role As Team Manager, you will provide strong operational leadership across the service, managing a team responsible for complex discharge planning. You will play a critical role in reducing Delayed Transfers of Care (DTOC) while ensuring residents and their families receive high-quality, person-centred support throughout their discharge journey. You will work collaboratively with health and social care partners, driving effective multidisciplinary working to achieve positive outcomes.
