Contract: Full-Time
Hours: 173 hours per month, including evenings, weekends and sleep-in duties
Pay: £12.71 per hour (Unqualified) | £12.92 per hour (Qualified) | £70 per sleep-in
We are recruiting for a Leaving Care Worker to support young people aged 16+ transitioning from care to independent living within supported accommodation and community-based settings.
This role involves delivering person-centred support to help young people develop practical life skills, improve wellbeing, access education, training, employment, and maintain suitable accommodation while preparing for adulthood.
Key Responsibilities
* Provide direct support to young people aged 16+ transitioning from care to independence.
* Deliver 1-1 support, keyworking sessions, and independent living training.
* Develop, implement, and review support plans in partnership with young people, families, social workers, and professionals.
* Support young people with life skills, budgeting, tenancy readiness, education, employment, training, and community engagement.
* Maintain accurate records, reports, case notes, and documentation in line with policies and regulatory standards.
* Follow support plans, risk assessments, behaviour management plans, and safeguarding procedures.
* Build positive working relationships with housing providers, education providers, employers, local authorities, and external agencies.
* Promote participation, wellbeing, social inclusion, and positive outcomes for young people.
* Work flexibly across different settings including evenings, weekends, and sleep-in shifts.
Requirements
* A caring, compassionate, and reliable approach.
* Good communication, organisational, and interpersonal skills.
* Ability to work independently and as part of a team.
* Understanding of safeguarding, confidentiality, equality, and professional boundaries.
* Flexibility to work evenings, weekends, and sleep-in duties.
* Previous experience within care, support work, youth work, leaving care, supported accommodation, or related settings is desirable but not essential, as full training will be provided.
